Understanding Tooth Sensitivity: Your Teeth Are Trying to Tell You Something

Tooth sensitivity—that sharp, sudden pain triggered by hot, cold, sweet, or acidic foods—affects around 1 in 3 adults. While it's incredibly common, it's not normal. Sensitivity is a symptom, not a condition in itself, and finding the underlying cause is key to lasting relief.

At our South Kensington practice, we don't just prescribe sensitive toothpaste and send you home. We investigate why your teeth are sensitive in the first place. Sometimes the cause is simple and easily addressed; other times sensitivity signals an underlying problem that needs treatment before it worsens.

The good news is that most causes of sensitivity are treatable. Whether you need a simple fluoride application, gum treatment, or restorative work, we can help you enjoy your favourite foods again without wincing in anticipation.

Why Teeth Become Sensitive

To understand sensitivity, you need to understand tooth structure. Your tooth has three layers:

Enamel

The hard, white outer layer. Contains no nerves—it's your tooth's protective shield.

Dentin

The softer, yellowish layer beneath enamel. Contains thousands of microscopic tubes leading to the nerve.

3

Pulp

The innermost layer containing the nerve and blood supply. This is what feels pain.

Sensitivity occurs when the protective enamel is compromised or the gum recedes, exposing the underlying dentin. The microscopic tubes in dentin (called dentinal tubules) act like tiny straws, transmitting stimuli directly to the nerve. Hot coffee, cold ice cream, acidic lemon—all trigger fluid movement in these tubes, which the nerve interprets as pain.

Common Causes of Tooth Sensitivity

Gum Recession

The most common cause. When gums pull back from teeth, they expose the root surface, which has no enamel protection. Recession can result from gum disease, aggressive brushing, grinding, or simply ageing. Root surfaces are naturally sensitive.

Enamel Erosion

Acidic foods and drinks, acid reflux, and certain medications can dissolve enamel over time. As the enamel thins, dentin becomes exposed. Learn more about dental erosion →

Tooth Decay

Cavities that penetrate enamel expose dentin to the oral environment. Sensitivity to sweets is often an early sign of decay. Learn more about tooth decay →

Cracked or Chipped Teeth

Cracks create direct pathways to the dentin and pulp. Sensitivity that comes and goes with biting pressure often indicates a cracked tooth.

Teeth Grinding (Bruxism)

Chronic grinding wears down enamel and can cause microfractures. Many people grind at night without knowing. Morning jaw pain and flat-looking teeth are telltale signs.

Aggressive Brushing

Brushing too hard or with a hard-bristled brush wears away enamel and damages gums. We often see "toothbrush abrasion"—V-shaped notches at the gum line—in conscientious patients who brush too vigorously.

Gum Disease

Periodontal disease causes gum recession and can destroy the bone supporting teeth. Exposed roots and loose teeth both cause sensitivity. Bad breath is often an early sign.

Recent Dental Work

Sensitivity after fillings, crowns, or whitening is common and usually temporary. If it persists beyond a few weeks, let us know—it may indicate a problem with the restoration.

Whitening Products

Both professional and over-the-counter whitening can cause temporary sensitivity. This is usually reversible, but overuse of whitening products can cause lasting damage.

Treatment Options

Treatment depends entirely on what's causing your sensitivity. Our approach is diagnostic first—we identify the underlying problem before recommending solutions.

1. Professional Fluoride Treatments (From £75)

High-concentration fluoride varnish applied in-clinic can strengthen enamel and reduce sensitivity by occluding (blocking) the dentinal tubules. We may also prescribe prescription-strength fluoride toothpaste for home use.

2. Desensitising Agents

Products containing potassium nitrate, arginine, or calcium phosphate can be applied professionally or used at home. These work by either blocking tubules or calming the nerve itself.

3. Composite Bonding (From £295 per tooth)

When sensitivity is caused by exposed roots, worn enamel, or abrasion defects, composite bonding can cover and protect the vulnerable area:

  • Covers exposed dentin and root surfaces
  • Repairs toothbrush abrasion notches
  • Completed in one appointment
  • Tooth-coloured and natural-looking

4. Gum Grafting

For significant gum recession, a gum graft can cover exposed roots permanently. This involves taking tissue from elsewhere in your mouth (usually the palate) and grafting it over the recession.

5. Night Guard for Grinding (From £495)

If grinding is causing sensitivity, a custom-fitted night guard protects teeth from further wear. This also addresses bite problems and TMJ symptoms.

6. Root Canal Treatment (From £950)

If sensitivity is severe and other treatments haven't worked—particularly if a tooth's nerve is inflamed or dying—root canal treatment may be necessary to remove the nerve and eliminate pain permanently.

7. Treating Underlying Decay or Damage

If sensitivity is caused by cavities, cracks, or failing restorations, treating the underlying problem—with fillings, crowns, or other restorations—will resolve the sensitivity.

Managing Sensitivity at Home

While professional treatment addresses the cause, these strategies can help manage symptoms:

Use Desensitising Toothpaste

Products containing potassium nitrate (like Sensodyne) can reduce sensitivity with regular use. For best results, apply a small amount to sensitive areas before bed and don't rinse.

Brush Gently

Use a soft-bristled brush and gentle pressure. Electric toothbrushes with pressure sensors can help prevent over-brushing. Brush in small circles, not aggressive back-and-forth scrubbing.

Avoid Acidic Foods and Drinks

Limit citrus, fizzy drinks, wine, and other acids that erode enamel. When you do consume them, use a straw and rinse with water afterwards.

Wait Before Brushing After Eating

Acids temporarily soften enamel. Wait 30 minutes after eating or drinking anything acidic before brushing to avoid abrading softened enamel.

Don't Clench or Grind

If you catch yourself clenching during the day, consciously relax your jaw. Keep your teeth slightly apart when at rest. Consider a night guard if you grind at night.

Is tooth sensitivity a sign of a serious problem?

Not always, but it shouldn't be ignored. Mild sensitivity from cold might just indicate thin enamel, while severe or constant sensitivity could signal decay, a cracked tooth, or nerve damage. The only way to know is through proper examination.

Why are my teeth suddenly sensitive?

Sudden onset sensitivity often has a specific cause: a new crack, recent dental work, aggressive whitening, a new toothpaste, or the start of decay. Gradual sensitivity usually indicates progressive wear, recession, or erosion.

Will sensitive toothpaste cure my sensitivity?

It can help manage symptoms, but it doesn't address the underlying cause. If you stop using it, sensitivity often returns. For lasting relief, you need to identify and treat what's actually causing the sensitivity.

Can whitening cause permanent sensitivity?

Properly supervised whitening causes temporary sensitivity that resolves within days. However, overusing whitening products—especially strong over-the-counter ones—can cause lasting damage to enamel. Always follow instructions and consider professional whitening for safer results.

My dentist can't find anything wrong—why do my teeth still hurt?

Some causes of sensitivity are subtle. Hairline cracks don't show on X-rays. Early recession may look normal. Bite issues causing stress fractures require careful analysis. If your concerns haven't been resolved, a second opinion with a detailed examination may help.
